Costs:

Initial consultation- $240 associated Medicare rebate applicable (No concession rates applicable)
Follow up consultations - $170 associated Medicare rebate applicable (No concession rates applicable)

WorkCover:

If this is part of a Workcover claim, please bring along your:
1. Claim number
2. Case manager details
3. Employer contact details (phone number and email) and advise them of your specialist appointment.

Please arrive 10-15 minutes early for your Appointment for registration and to fill in a health questionnaire.

When you come for your Appointments, please remember to bring the following

• Private Health Insurance information
• Medicare card
• Referral Letter
• List of medications (if any)
• Workers Compensation details (if applicable)

To cancel an Appointment

Telephone the office during business hours and allow at least 1 days’ notice so that we can offer your appointment time to patients on our waiting list.

We recognise that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologise if we keep you waiting.
